Ventilatory and cardiocirculatory exercise profiles in COPD: the role of pulmonary hypertension.

CONCLUSION:Patients with severe PH showed an exhausted circulatory reserve at the end of exercise. A profile of circulatory reserve in combination with ventilatory impairments was found in COPD-patients with moderate PH or no PH. The results suggest that pulmonary vasodilation might only improve exercise tolerance in COPD patients with severe PH. PMID: 22556320 [PubMed - as supplied by publisher] (Source: Chest)

Updates in oral appliance therapy for snoring and obstructive sleep apnea

Conclusion  Consistent follow-up and management are needed to increase clinical success rates in OA therapy for OSA. Depression but not sleep disorder is an independent factor affecting exacerbations and hospitalization in patients with chronic obstructive pulmonary disease

Conclusions:  Depression and sleep disorders are very common comorbidities among COPD patients, and significantly reduce activities and HRQOL among these patients. Depression, but not sleep disorder, is an independent risk factor for exacerbations and hospitalization among COPD patients.© 2012 The Authors. Influence of age on the clinical differentiation of atypical pneumonia in adults

Conclusions:  These results indicate that it is difficult to distinguish between atypical pneumonia and bacterial pneumonia in the elderly, using the JRS scoring system. When treating patients aged ≥60 years, physicians should use fluoroquinolones or β‐lactam antibiotics + macrolides as empirical first‐choice drugs, so as to always provide antibiotic protection against potential atypical pathogens.© 2012 The Authors. Determination of Tobacco Smoke Exposure by Plasma Cotinine Levels in Infants and Children Attending Urban Public Hospital Clinics [Article]

Conclusion  In a city with a low smoking rate (12%) and public smoking bans, we documented 55% exposure among infants and young children, using a plasma biomarker, compared with 13% exposure reported by parents. Because SHS is associated with significant respiratory diseases and parents underreport exposure, routine biochemical screening should be considered as a tool to identify and reduce SHS exposure.
