"Walking with Only Noninvasive Ventilation in Stable Hypercapnic COPD Patients: Sufficient or not?"

"Walking with Only Noninvasive Ventilation in Stable Hypercapnic COPD Patients: Sufficient or not?" COPD. 2016 Mar 25;:1 Authors: Walker DJ, Windisch W, Dreher M Abstract We are grateful to Ulasli and Esquinas for their comments to our paper.. They argued that arterial blood gas analyses were not performed without oxygen prior to the 6-minute walk test with noninvasive ventilation (6MWT-NPPV). This point has already been discussed in our original work by indicating the limitations of our study. The reason for using oxygen prior to exercise testing was to guarantee comparable starting conditions. The role of thoracic surgery in the management of mesothelioma: an expert opinion on the limited evidence.

Authors: Bertoglio P, Waller DA Abstract Surgery has a key role at different points in the management of Malignant Pleural Mesothelioma. Diagnosis with video assisted thoracoscopy offers excellent sensitivity and specificity and a direct view of the pleural cavity to verify the extent of the tumor. Nodal involvement can be assessed by mediastinoscopy and either talc pleurodesis or partial pleurectomy can be used for symptom control in advanced stage disease. Extra Pleural Pneumonectomy (EPP) and Extended Pleurectomy Decortication (EPD) are used to prolong survival although the benefit of radical surgery has not has been fully clarified; EPP failed to show its benefit in the MARS trial and EPD is currently under investigation in the MARS2 trial. More randomized prospective trial dat...

Modification of the GOLD recommendations for chronic obstructive pulmonary disease to broaden their usage in Turkey.

Authors: Gunen H, Kilinc O, Polatli M, Suerdem M, Uzaslan E Abstract Despite the introduction of numerous national and international COPD guidelines designed to provide clinicians with optimal evidence-based disease management strategies, COPD remains an underdiagnosed and poorly treated disease. The Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines recommend disease specific diagnosis and management strategies, and they are widely used internationally. In this short report we present the outcomes of a meeting of respiratory experts in Turkey who reviewed the GOLD guidelines and their applicability to the Turkish healthcare system. In particular, we were interested to investigate the possibility of developing a simplified version of the combined assessment mo...

The Treatment of Allergic Respiratory Disease During Pregnancy.

This article reviews the recognition and management of asthma and allergic rhinitis during pregnancy, paying close attention to the general principles of allergy and use of asthma medication during pregnancy. Both allergic rhinitis and asthma can adversely affect both maternal quality of life and, in the case of maternal asthma, perinatal outcomes. Optimal management is thus important for both mother and baby. This article reviews the safety of asthma and allergy medications commonly used during pregnancy.
